Aged Care On-site Pharmacist Program (ACOP)

From Thursday, 25th July, Lions Haven will be participating in the Australian Government's Aged Care On-site Pharmacist (ACOP) Program.

Dear Residents and Families,

We are pleased to let you know that Lions Haven for the Aged will soon be introducing a new initiative to further support resident health and wellbeing. From Thursday, 25th July, we will be participating in the Australian Government’s Aged Care On-site Pharmacist (ACOP) Program. 

As part of this program, a qualified on-site pharmacist will be working at our facility every Wednesday and Thursday. 

The pharmacist will play a key role in supporting the safe and effective use of medications. They will work closely with residents, families, staff, doctors and other healthcare professionals to ensure medication-related care is well-coordinated and tailored to each resident’s needs.

The pharmacist can assist with: 

  • Reviewing medications to help ensure they are safe and appropriate
  • Supporting staff and doctors with medication management
  • Helping identify and prevent medication-related problems
  • Providing education and information about medications
  • Supporting smooth transitions of care, such as after a hospital stay

We encourage residents and family members to speak with the on-site pharmacist if they have any questions or concerns about medications. They are here to help and are a valuable part of our care team.

We’re excited to welcome this new role to Lions Haven and are confident it will enhance the quality of care we provide to our residents.
